Diabetes Self-Management Check List Print E-mail

Check List for Diabetes Self-Management:

1. Have your eyes checked at least once a year, including a dilated eye exam

2. Visit your medical doctor on a regular basis at least every 6-12 months for a physical check up (bring your glucose numbers with you).

3. Monitor your blood glucose at home. Medicaid, Medicare and some insurance carriers may the machines and supplies easier to get these days.

4. Check your feet on a daily basis for sores, blisters, and red areas. See your doctor as soon as possible with any problems.

5. Eat the right foods in the right amounts and do not skip your meals.

6. See your dentist at least yearly, preferably every 6 months.

7. Keep your blood pressure under control.

8. Take your medications faithfully.

9. Have your glucose, Hgb A1-C, and lipids checked on a regular basis.

10. Look for diabetes support groups in your area, and self-management education courses that may be available by going to www.aadenet.org, or call your local health care facility.
